This 56 y/o male with back pain was referred for MRI. Selected images shown above demonstrate spinal epidural lipomatosis. The thecal sac has the typical "Y" configuration on axial section at the L5 level. We consider this an idiopathic case as the patient was not on steroids, was not obese, and had no other known predisposing conditions.
