This 84 y/o hypertensive male was brought to the Emergency Deparment with hypotension and chest pain. Selected images from his chest CT shown above demonstrate mediastinal emphysema, a large amount of retrocrural air, and pleural fluid. The combination of findings is consistent with esophageal rupture - Boerhaave's syndrome.
