This 24 y/o male with a history of chronic heel pain was referred for MRI of the ankle. Selected saggital images shown above demonstrate findings consistent with an intra-substance (interstitial) tear of the achilles tendon with tendon thickening and linear increased signal. There is fluid in the retrocalcaneal (pre-achilles) bursa and there is edema in the posterior calcaneus.
