This 53 y/o male with a history of acute myelogenous leukemia presented with acute onset of ataxia. Selected T1, T2 and post gadolinium MRI images are shown above. There is an enhacing mass in the region of the roof of the fourth ventricle. The findings are compatible with a granulocytic sarcoma (chloroma). The lesion resolved with radiation and chemotherapy.
