This 67 y/o male with an abdominal mass was referred for abdominal CT. Selected pre and post contrast images are shown above. There is a large left adrenal mass which contains some calcifications and demonstrates heterogeneous enhancement. There is a hepatic metastasis and likely an early metastasis to the right adrenal. A non-functioning primary adrenal cortical carcinoma with hepatic metastasis was confirmed by biopsy and endocrine workup.
