1. Are you willing to participate in the BYU project?
2. Do you agree to pay the $150.00 testing fee? This fee will include a test report, test table, and explanation of the test results.
3. Include information, if possible, on your ancestry. Who was your earliest known ancestor?
4. Include you Name, Address, Telephone Number and E-Mail address so that you can be contacted as details become available.
Remember that only male's may participate as they alone carry the necessary Y-chromosome.
