NAUCC 2004 Registration

All Competitors and Non-Competitors

Please print / download and fill out one registration form per person. The blank registration form may be reproduced as needed. Payment must be submitted with each form or group of forms via check, money order or bank check in US funds only payable to Unicycling Society of America, Inc.

Group Freestyle Competitors

All members of each group must be registered as competitors for NAUCC 2004. Each Group must have one designated Contact Person. The Group Contact Person should print / download and fill out one of the forms below and turn it in along with a complete roster for their Group to convention registration officials no later than the close of registration on July 13th.

Club Show Competitors

All members of each Club Show Unit must be registered as competitors for NAUCC 2004. Each Club Show Unit must have one designated Contact Person. The Club Show Unit Contact Person should print / download and fill out one of the forms below and turn it in along with a complete roster for their Club Show Unit to convention registration officials no later than the close of registration on July 13th.

Parade Competitors

All members of each Parade Unit must be registered as competitors for NAUCC 2004. Each Parade Unit must have one designated Contact Person. The Parade Unit Contact Person should print / download and fill out one of the forms below and turn it in along with a complete roster for their Parade Unit to convention registration officials no later than the close of registration on July 13th.

All members of each Hockey Team must be registered as competitors for NAUCC 2004. Each Hockey Team must have on designated Contact Person. The Hockey Team Contact Person should print / download and fill out the form below and turn it in along with a complete roster for their Hockey Team no later then the close of registration on July 13th.
