New to MIPT?

In California, a licensed physical therapist may provide consultation services, and evaluate and treat a person without a prescription from a physician. Any person may obtain a wide variety of medically oriented and/or wellness services from a licensed physical therapist. Insurance companies, however, determine the circumstances under which physical therapy services are covered. Lise McCarthy, PT, MA, GCS, is a Fee-for-Service Provider and a Medicare Provider.


Medicare requires that a patient be under the care of a physician if physical therapy services are to be a covered benefit. MIPT will need to verify that Medicare is the primary insurance company and will request the following pieces of information: the patient's name, date of birth, and Medicare number. If the patient is incapacitated, or otherwise unable to sign and complete the required forms, MIPT will ask that the responsible party do this on the patient's behalf.

Becoming a New Patient and About Your First Visit

All forms will need to be completed and signed before your first visit. These forms can be faxed or mailed to you.


The evaluation process begins when we first start talking with you on the phone and is completed after Lise identifies the causes of the problems and what can be done to help you or your loved one live better or more independently. Before performing any test, she will first describe the test, answer your questions and obtain your verbal consent to be tested. Most evaluations take no more than an hour. In complicated or unusual cases, sometimes the evaluation process is spread over several visits.
