We will be glad to give you a completed insurance form to file with your insurer.

Please bring your insurance card, or a copy of both the front and the back, to your first meeting.

We can discuss other concerns you may have about your insurance coverage such as “Seeing an Out-of-Network Provider,” or “Point of Service Option” when we meet.

Payments

Mental Health treatment is an investment in yourself and your future. We can discuss any questions or concerns you may have about payment arrangements when we meet. Unless other arrangements are made, payment is expected at the time services are rendered. Until insurance reimbursement begins, payment – by cash, check, or credit card – is expected at the time services are rendered. We accept MasterCard, Visa, checks or cash.
