ORDER FORM


To order, simply describe the recording on the lines 
below and enter the amount indicated on the home page/index.  
After you have described all the titles you wish to order, 
simply total up the cost of all and apply the following 
postage rates:

Domestic US: 

1-3 CDs
Special 4th., no insurance  $2.00

4-7 CDs
Special 4th., no insurance  $4.00
                      
8-12 CDs 
Special 4th., no insurance  $6.00
                      

For 1st Class add $1.00 to each of the above $ amounts.

International:  

Surface mail, $1.25 per CD, minimum $5.00 or 4 CDs.

Air mail, $2.25 per CD, minimum $4.50 or 2 CDs

After totalling the cost of the CDs and the postage, 
if you are a California resident, add 7.25% for sales tax.

Note:  There is no way to order directly over the Internet.  
You must send payment by check with your order.
  
The order form provided here is simply for your convenience.  
You  need to print it out and mail it to us in order to use it. 
You do not need to use it to order so long as you send the 
proper amount for the cost of the CDs, postage and tax, 
if applicable
