I will deliver all of the necessary equipment and supplies to your location to provide a safe, comfort filled experience during your session.
 
After I arrive for your first session, I will ask you to complete a client information and health history form. I will assess the health history and conduct a brief interview (this time will be scheduled and of no charge to you).  The contact information will only be used in the event I would need to reschedule a future visit or for appointment confirmation and emergency contacts. The health history will assist me in determining if any health conditions, injuries, medications or allergies would be of relevance to the session. If a certain condition would warrant it, I may require a doctor's release before massage could be performed.  I will never use any of your information except for your benefit and without your prior written consent.
 
During the interview process you will have input on the session structure, and after you have made a listening selection, I will leave the room and allow you to disrobe and assume a comfortable position between the sheets on the table. Your modesty will always be protected and only the area being worked on will be uncovered. I will check in frequently for your comfort level and to make sure the pressure being applied is appropriate. You may talk if you wish as this is your time, but allowing yourself to become immersed in the relaxation of the massage will yield optimum results. When the session is complete, I will exit the room and allow you time to become fully alert and get dressed. 
For elderly clients, I may need to assist the client when getting up as massage can have a profound lowering effect on blood pressure. 
 
  

 Policies
  1. On subsequent visits, it is the responsibility of the client to keep the massage therapist informed of any changes in medical treatment, medication, or new injuries, and if warranted provide permission from the appropriate health care professional so that massage may continue.
  2. The client must be properly and modestly draped at all times.
  3. Payment is expected at the time services are rendered. I do not file claims for medical insurance, but will provide documentation to assist the client if he/she chooses to do so.  A sliding fee scale and/or payment arrangements may be considered for those providing evidence of significant financial hardship.
  4. All clients will be notified of Fee for Service increases within one week or at least 72 hours prior to an appointment. Fees will be evaluated yearly and adjusted in relationship to the market.
  5. Appointments must be canceled 12 hours in advance. Two failures to do so will result in the client not being rescheduled and being charged for the session.
  6. Improper behavior of any kind will not be tolerated and if such occurs, the massage will end and payment will be made in full and the client / massage therapist will leave. Furthermore, a Termination of Session report will be filed and the client will not be rescheduled or referred to another massage therapist.
  7. It is recommended that the client see a physician for any physical condition or injury that he or she may have.
