- All patients, or parents of minor patients, will be asked to
complete the brief medical history form shown below. The information requested is
necessary to insure proper treatment. All information will be regarded as
confidential. Researching this information prior to your first appointment will
expedite your treatment.

- To simplify your check-in when you arrive at the
office, you may print the Health History (both pages) in advance, fill
them out, and bring them with you to your appointment.
