FEES AND PAYMENTS 
- Fees:
- Dr. Evans' fees are usual, customary, and reasonable. Fees are determined by the
difficulty of the tooth to be treated. If endodontic treatment is indicated and
decided upon, the fee quoted will include all necessary x-rays, all routine office visits
needed during normal endodontic treatment, routine post-operative treatment, and
recalls. Any circumstances requiring additional procedures (such as post and core
buildups or additional diagnostic appointments, etc.), or any visits other than during
normal office hours are subject to additional charges.

- Payments:
- If you have no dental insurance
- Payment in full for services rendered is expected by completion of
the treatment. If the endodontic treatment can be completed in one visit, payment
will be expected at the completion of the visit. If two or more visits are
anticipated, half of the fee will be due when the treatment is begun, and half will be due
upon completion.
If you have dental insurance
- We are happy to process your insurance for you. With the
help of our comprehensive computer data base of insurance carriers and benefits, the
portion of our fee we anticipate your insurance carrier to cover will be estimated.
We ask that you pay the remaining portion at the beginning of treatment. When the
insurance company pays on your behalf, we will reimburse you if your anticipated portion
already paid was excessive. We will bill you if your anticipated portion already
paid was not enough.

- If you need financing
- We offer CareCredit. Call the office for available options.
- Remember, Dr. Evans renders service to you, not your insurance
company. Ultimately, you are responsible for Dr. Evans' entire fee.
We accept MasterCard, Visa, CareCredit, personal checks, and cash.
