· Developed Certificate of Need application
for Department of Health approval of a proposed Assisted Living facility. The
facility would be developed under new construction by a major health system providing a full continuum of neonatal to eldercare
health services. Identified a unique population-based methodology for forecasting
service area need for assisted living units. The methodology incorporated consideration
of functional health status, including activities of daily living measures by
age and gender, and financial categories of projected household income in order to evaluate the potential market for the proposed
service.
· Generated
grant writing deliverables for funding applications in the governmental, NGO public, private and nonprofit sectors. Guided research and other funding applications through the preparatory, review and submission processes,
organizing collaboration with colleagues and third party affiliates.
· Developed a 3-year County grant application for State funding
to establish a comprehensive Tobacco Control Program. The plan incorporated CDC’s
national tobacco control goals and objectives, along with multi-disciplinary commitment and participation of a broad range
of County community, govenmental and professional stakeholders. The application
included a review of and recommendation for the most successful on-going State-based program models across the country. Grant funding was successfully secured.
