Burke County Health Check Coordinator
Hi, my name is Gwen Stephens, and I am the Health Check Coordinator for Burke County. I have been in this position
since the inception of the Health Check program, so I am very familiar with the services that are available to you. The objective
of our program is to make sure all of our children have child physicals and are immunized at appropriate times, and in order
for us to do this, we often make phone calls or send notes reminding parents that it is time to make the well check appointment.
As your Health Check Coordinator, I make every effort to assure that all children in the Health Check Program (Medicaid) receives
regular well child checkups and childhood immunizations (shots). Well check-ups and immunizations are so important to your
child's good health for the future. As your local Health Check Coordinator, I can assist your family with acquiring transportation
services, WIC, Carolina ACCESS CARE, NC Health Choice (medical coverage for children 6-10 who are not eligible for Medicaid,
making physician's appointments, finding a doctor, and finding a dentist. These are just a few things that your Health Check
Coordinator does in Burke County.
