Team Entry Form

 

Please use this entry form if you are registering an entire team online.  All information is optional.  You can always add indviduals later with the single entrant registration.  Also, just send an email if you have any changes or questions.

Instructions:

• We encourage all teams to have approximately 8 participants.  We go all day and need to factor in fatigue, injury, and inability of players to attend every game.
• Uniforms are encouraged.  All players should at least wear similar shirts.
• If you add or subtract players from your team, just send an email to erniehoops@earthlink.net, reference the team name, and provide updates.
• Entry is tax deductible and is $50 per player.
• Waivers must be signed by everyone.  Those under 18 must have the waiver form signed by a guardian.  You can print the waiver form on the web site.
