  • Entry is tax deductible and is $50 per player.
  • Please R.S.V.P by October 15 by calling Coach Russell White at 818 222-7177  x733, or register online.
  • Waivers must be signed by everyone.  Those under 18 must have the waiver form signed by a guardian.  You can print the waiver form on the web site.
