Do You Need Mental Health Counseling?

Rate yourself according to how much impact your current problems have on your ability to function in normal daily life. The numbers at the end of each answer option indicate level of interference with normal life; the higher the number, the more troublesome the impact. One option for each question is provided if there is little to no impact on your life at all. I have worries and fears that:
I tolerate well, are momentary or fleeting, and cause little if any side effects for me [1]
occupy my thoughts too much, but with effort I can refocus [2]

suicide attempts, other extreme violence, being incoherent, having inappropriate responses [6]

serving me well in my commitment to move forward with my goals and dreams [1]

seriously dysfunctional or disruptive, and cause lots of trouble for me and others [4]
show very poor judgment, very faulty thinking or really bad communication skills [5]
indicative of a major impairment in personal functioning, repeated personal endangerment [6]
Relationships or situations of the past or present:
have had their normal tensions, but are overall satifying and rewarding for me [1]
cause me some concern or frustration or lost sleep, but I can usually work things out [2]
have been unhappy, unsatisfying, and I dwell on what went wrong much of the time [3]
have damaged me so much that it's hard to make or keep friends, or a job [4]
make me extremely defensive or abusive, or cause me to neglect my responsibilities [5]
traumatized me so much that I have attempted suicide, or I've totaly withrawn from the world [6]
