SUPPLEMENTAL INFORMATION TO ASSIST YOU IN PREPARING FOR YOUR
APPLICATION AND IN COMPLETING THE BIOGRAPHIC DATE FORM OF-230 PART
I
PASSPORTS: Passports can usually be obtained through the Embassy
of your country in Washington, D.C. or Ottawa, Ontario, or through a
Consulate General in a major city such as New York or Montreal.
Or go to your local passport office.

These are NOT available for same day pick-up, they will mail the completed passport to you
in 10 days or so. I think there was the option of sending in for one by mail but I'm not familiar with it.
If you call the above number they can give you the details.
BIRTH CERTIFICATES: Civil birth certificates are required.
Birth certificates must show the names of both parents. The following
applies to certain places of birth:
- PROVINCE OF QUEBEC: Civil Documents are required and must be
obtained from Direction de l'Etat Civil in Montreal (2050 rue de Bleury,
Montreal, Quebec H3A 2J5, tel: 514-864-3900) or Quebec City (205
Montmagny, Quebec, Quebec G1N 2Z9, tel: 418-643-3900). The document must
show the names of both parents. A nominal fee will be charged.
- PROVINCE OF ONTARIO: Apply to the Office of the Registrar
General, P.O. Box 4600, Thunder Bay, Ontario P7B 6L8, tel: 807-343-7420
Call NO CHARGE 1-800-461-2156.
The LONG FORM certificate with the parents names should be
obtained. The short form is only acceptable if accompanied by a letter
from the Registrar stating that the long form cannot be issued.

Took about a 1/2 an hour. I paid an extra 15 dollars to get it the same day, so my cost was
37 dollars for EACH copy. They are available by mail, call the number above for details.
- OTHER PROVINCES IN CANADA: Civil documents are required. the
certificate must show the names of both parents.
- ITALY: You must request a birth certificate which includes
the names of both parents issued from the Office of Vital Statistics of
the town where you were born.
- OTHER COUNTRIES: Information regarding procedures to obtain
birth certificates is usually available from the Embassy or Consulate of
the country concerned. If other inquiries are unsuccessful, you may
contact this office for additional information.
UNOBTAINABLE BIRTH CERTIFICATES: In those rare cases in which
it is impossible to obtain a birth certificate, the Consulate General
will consider secondary evidence which is original and
contemporaneous. Such documents will not be acceptable, however, if
information available to the consular officer indicates that birth
certificates are usually available from the country of your birth. If
a baptismal certificate is submitted, it must contain the date and
place of birth and the names of both parents. Baptism must have taken
place shortly after birth and the certificate must include the seal of
the church or issuing authority.
ADOPTION CERTIFICATES: If you were adopted, you must submit
the original or a court-certified copy of the adoption decree.
MARRIAGE CERTIFICATES: Civil marriage certificates are
required. The following will be useful to persons married in:
- PROVINCE OF ONTARIO: Apply to the Office of the Registrar
General, P.O. Box 4600, Thunder Bay, Ontario P7B 6L8. A fee is charged
for each copy.
- PROVINCE OF QUEBEC: Apply to either the Direction de l'Etat
Civil in Montreal (rue de Bleury, Montreal, Quebec H3A 2J5, tel:
514-864-3900) or Quebec City (205 Montmagny, Quebec, Quebec G1N 2Z9, tel:
418-643-3900). A nominal fee will be charged.
DIVORCE DECREES: You must submit the original or a court-certified
copy of the FINAL divorce decree for all previous marriages.
If you don't have a certified copy of the final decree contact the lawyer who handled it for you,
or the courthouse where it was issued.
POLICE CERTIFICATES: Must be obtained, covering ALL PREVIOUS
PLACES OF RESIDENCE OF TWELVE MONTHS OR MORE SINCE ATTAINING THE AGE OF
16. The certificate issued for your present place of residence must
have been issued within the 12 months preceding the date of your final
visa appointment in Montreal. The following information applies to
specific jurisdictions:
- CANADA: The only acceptable police report from Canada is the
certificate issued by the Royal Canadian Mounted Police (RCMP). If you
live in the Montreal area, you should apply directly to the RCMP at 4225
Dorchester Blvd. West, Montreal, Quebec, week days, except legal holidays,
between 9:00am - 11:00am & 1:00pm - 3:00pm. If not residing in Canada,
you must send a duplicate set of fingerprints (taken by the local police)
to the Commissioner, RCMP, P.O. Box 8885, Ottawa, Ontario K1G 3M8, ATTN:
Identification Branch. The fee is CN$26.00.
In ONTARIO call 1-800-472-4848. They will provide you with the address of the closest
facility to get your RCMP clearance. They gave me the following 2 places close to me:

Note: Make sure they understand you do not require a FINGERPRINT clearance, which can take 4-5 weeks I understand.
You need "the other one" that only takes from 15 minutes to a few days depending on which office you go to.
- UNITED STATES: Do not obtain police certificates from places
in the United States unless you have an arrest record there. A certified
copy of the court record indicating the disposition of the proceedings
will expedite processing of your immigrant visa application.
- GREECE: The only acceptable police certificate from Greece is
know as the Pinikon Metroon, Type B. When applying, Type B (Piires
Antigrafon Tipou Beta) MUST be specified. Persons outside Greece should
apply in writing, including full details of their identity, which must be
certified by the Greek Consulate serving the area in which they now
reside. A fee is charged for each copy.
- OTHER COUNTRIES: Procedures vary from area to area. Please
consult your own Embassy or Consulate for information. If this is
unsuccessful, you can call or write to this office.
- HAITI: ALL civil documents (e.g. birth, marriage,
divorce or death certificates) should be issued by the National
Archives, not by local offices.
