Physical Examinations for Immigrant Visa Applicants
Please Read Carefully


Each Immigrant Visa Applicant, including children, regardless of age, must have a medical examination. Medical examinations may only be done by one of the specially appointed panel physicians listed on the reverse of this form. You cannot have the examination done by your family physician or any other doctor not listed on this sheet. Please telephone one of the desiginated physician's offices to arrange your medical exam. Please give the doctor the date of your visa appointment or explain that you are a fiance(e) visa applicant. The panel physician will advise you regarding the cost of the exam and tests. NOTE: You must bring your passport or other photo-identification and your appointment letter to the doctor's office, radiologist and lab where your blood test is taken. On the date of your VISA APPOINTMENT at the consular section, bring your x-ray and unopened medical report to this office, Place Felix Martin, 1155 St. Alexandre Street, Montreal.

The examination will include a medical history, physical examination, chest X-ray and blood tests for syphilis and HIV (Human Immunodeficiency Virus). The physical examination will include at a minimum examination of the eyes, ears, nose and throat, extremities, heart, lungs, abdomen, lymph nodes, skin and external genitalia. You may have the examination even though you are having your menstrual period. Children 15 years of age do not routinely require a chest X-ray or blood tests.

United States Public Health Service regulations require pregnant women to have a chest X-ray. It is possible to perform the X-ray during pregnancy with proper shielding of the abdomen. However, if you are pregnant and do not wish to have the X-ray, a temporary exemption may be granted. To be granted the exemption, you must bring with you to your appointment, written certification of your pregnancy signed by your personal physician or obstetrician. If you had syphilis you must present the panel doctor with a written certificate, signed by a doctor or public health official, proving that you were adequately treated. If you have ever had a positive VDRL or other blood test for syphilis and were not treated, please bring a written explanation signed by your doctor

If you have ever had a positive skin test for tuberculosis, please bring a certificate from your doctor, outlining the circumstances surrounding the positive tests result and, indicating what, if any, treatment you were prescribed and for how may (sic) months you took the treatment. If you have ever been diagnosed as suffering from tuberculosis, you must present a written certification, signed by your doctor, to prove that you were adequately treated. The certificate must include dates and types of medications taken. If you have ever had an abnormal chest X-ray, please arrange to borrow the last Xray films taken of you and bring them to the Visa Medical Appointment. The actual films, not the typed reports, may be required to compare with the X-rays that will be taken at the Visa Medical Appointment.

Applicants treated or hospitalized for psychiatric or mental illness, alcohol or drug abuse, must present written certification which must include the diagnosis, duration of treatment rendered, and prognosis.

Applicants with mental retardation or learning disabilities, must present a report outlining their problems and any special requirements as far as education or supervision that may be required.

If you have had any history of harmful or violent behavior resulting in injury to people or animals, or harm to inanimate objects, you must provide information which will allow the doctors to determine if your behavior was related to any psychiatric or medical problem, or to drug or alcohol use. Harmful behavior includes attempted suicide or harm to yourself no matter how minor in nature.

Applicants being treated for chronic medical problems, or those taking medications on a regular basis, should be familiar with the medical conditions that they are suffering from, and the names of the medications they are taking. If you are not sure of the diagnoses, please have your doctor prepare a certificate outlining your problems, the current treatment, and prognosis. Please bring a list of any medications that you may be taking.

AIDS NOTE: As part of your examinations, a blood test for the antibody to Human Immunodeficiency Virus (HIV) is required. The results of a positive test must be reported to the consular officer and to the appropriate Canadian public health authorities.
