Museum of Sex - or MoSex. In New York. A privately funded museum which may eventually have a library. They recently mounted their first display, and published a book.
Museum Erotica - in Copenhagen. I visited in early 2000, and again in 2004 and 2005. I've really enjoyed the multicultural and historical aspect. There were many artifacts that I have seen again or discussed in different sexology books. Fun.

Beate Uhse in Berlin, Germany. Museum for the sex-positive toy store. In German.
Erotic Museum - One of two erotic museums in Amsterdam.  Several floors of cool, including a nearly-functional "Bicycle Built for Screw." They had a great gift shop where we purchased numerous silly figures. There is also another Erotic Museum in A'dam, the "Venus Temple," which doesn't have a website.
