OVERVIEW

Rehabilitation project to contribute in improving the health service for populations in Bantul continues. Second Visitation by Panos Katrakis, Merlin Indonesia Operational Coordinator, early March 2007 made our project controlled and improved. Thanks for his time and effort.

Up until this update four of the Pustu have been reactivated. Health-providing activities in all 4 Pustus are now conducted in a regular basis. Direct socializations regarding Pustu’s operational condition are constantly performed during team’s regular escort to the sub villages. All of the assisted pustus are also equipped with daily reporting forms completed with patient’s residence; this data has assisted Pustus to evaluate their coverage area in reaching for the community. Monthly accumulation of the data are  compiled and reported to the Puskesmas. The total of patients treated through Pustu Assistance activity during the month assignation was 717 patients. Posyandu appliances distributed based on prior needs-assessment - the items distributed comprise of growth monitoring devices (scales, measuring tape, adaptable microtoa and wooden baby holder for baby’s scale); tables; plastic chair; cupboard and Educative Playing Toys.

The basic functions for posyandus are to monitor child’s health, growth and development; evaluate family’s general condition through mother escort; assist village’s health staffs in monitoring pregnant women and participate in community hygiene and health activity in general, the team is projecting these ideals through its three days training of trainers (TOT). The activity was held in Jogjakarta Plaza Hotel on 28-30 March 2007 with the
assistance of Bantul DHO’s MCH division (the training was opened by Head of Bantul MCH division, drg. Dewi Marhaeni, MSi.) and Gadjah Mada University (Medicine and Psychology Faculty).

ACTIVITIES
Pustu Assistance

Pustu’s Infrastructural Support
To complete the Puskesmas’ requirement, on 6 March 2007 our team has managed to provide Puskesmas Bambanglipuro with 550 L water tank and 4 M water tower. The support was conducted after previous coordination with the head of Puskesmas. During previous coordination the head of the Puskesmas Bambanglipuro implied the need of water tank and tower to provide water for the new Puskemas building.

Pustu’s Medicinal Support
The appliances distributed are combined with DHO’s and local Puskesmas’ medical and equipment allocation for Pustu. The main objective of the activity is to complete the medicinal need required for Pustu’s operational status and expected to optimize Pustu’s health-providing activity. During March, distributions of the appliances were performed on 7-24 March 2007.


Pustu’s Health Services Assistance

The total of patient treated through Pustu Assistance activity during the month assignation was 717 patients. In Pustu Temuwuh, Dlingo Sub District, we assisted Primary health service activity conducted by the Puskesmas authorized. During the month (March 1-31, 2007), the Pustu has managed to treat 173 patients. In Pustu Jatimulyo, Dlingo Sub District, We assisted Primary health service activity conducted by the Puskesmas authorized. During the month (March 1-31, 2007), the Pustu has managed to treat 236 patients. In Pustu Wukirsari, Imogiri Sub District, During its prior operational weeks (March 1-31, 2007), the Pustu has managed to treat 98 patients. And in Pustu Mulyodadi, Bambanglipuro Sub District during the month (March 1-31, 2007), the Pustu has managed to treat 210 patients.

Daily Surveillance and Morbidity Report
During March 2007 our team has managed to compile and compose the daily surveillance and morbidity report. The surveillance reports made were also reported to the local Puskesmases. From all catchments visitation to pustu temuwuh, Jatimulyo, Wukirsari and Mulyodadi, there were: 20 Acute watery Diarrhea, 174 Acute Respiratory Infections, 220 Mother and child health (ANC, Contraception, Immunization), 1 Typhoid fever and 302 others Visitation.

Posyandu Assistance

In order to evaluate and optimize health activity conducted for the community our team performed regular escort for each of the Posyandu’s monthly activity. The escort also proved to help to improve cadres' self esteem in conducting health services for their surrounding. Improvements in Posyandu’s overall activities also describe the positive feedbacks of our month's escort.

Monthly Weighing Scales for Children U5
In Sumbermulyo Village, Bambanglipuro Sub District, we assisted 16 posyandus during the month activity, In Mulyodadi Village, Bambanglipuro Sub District, we assisted 14 posyandus, In Wukirsari Village, Imogiri Sub District, we assisted 18 posyandus, In Temuwuh Village, Dlingo Sub District, we assisted 13 posyandus, and In Jatimulyo Village, Dlingo Sub District, we assisted 10 posyandus during the month activity.

The overall presence of children < 5 y.o. attending the activity was 74,74 %.  Evaluation regarding the attendance and other related factors are continually commenced. Coordination with the local authorized (village midwife, community leader and Puskesmas’s staffs) was also conducted to fulfil the community’s requirement for health guide and escort. The coordination can also assure maintenance of future activity.

Antenatal Care
During the month of February 2007, ANC treatment has been performed in 54 Posyandus, with total catchments of 161 pregnant women. Overall achievement for the program was 47,6%.

Posyandu Appliances Support

To facilitate cadres’ health-promoting activity our team also supports the cadres with blank flipcharts, markers and other related equipment. Appliances distributed were based on prior needs-assessment, the items distributed comprise of growth monitoring devices (scales, measuring tape, adaptable microtoa and wooden baby holder for baby’s scale); tables; plastic chair; cupboard and Educative Playing Toys.

Posyandu appliances distributions were conducted on 1-5 March 2007 while the health-promoting material is being distributed along with Posyandu Job Training activity.




Flipchart Distribution for Posyandu

All 71 Posyandus will receive 4 flipcharts, each with a different category. Our team will produce 4 key messages about: Avian Influenza, Diarrhea, Dengue Haemorrhagic Fever and Hygiene Promotion.

Leaflet and Posters Disseminations
To support and complete the health promoting activity conducted in our working villages. Our team produce standardized leaflet on community hygiene on earthquake affected environment (PHBS), Preventing Dengue and Diarrhea. Our team produced 1000 leaflets for each topic, with total of 3000 leaflets. The entire leaflet has been distributed to each village through the Pustus on 22-29 March 2007. For Posters, our team generated 4 types of posters on Avian Influenza, Dengue, Hygiene Promotion and Diarrhea. We produced a total of 2000 Posters and plan to distribute them in the early weeks of April.

Posyandu Job Training and Community Health Promotion
During the month the team has able to escort 18 Posyandus and facilitate the community health education activity. The occasion took place in Posyandu and involved community leaders for further implementation. The cadres were encouraged to conduct health promotion with our team escort as support. During the activity the attendance received material handouts related to the topic presented. Overall community who attended the gathering was 880 persons.

Capacity Building for Cadres
Entering the last month of the program our team modified cadres’ training to outdoor motivating activity. The main objective is to build cadres' leadership role, self esteem, and increase their motivation. The activity was held in Kedung Sriti (one of the Jogjakarta camping sites) on 26 March 2007. The training was held by dividing cadres into smaller groups (lead by a facilitator) to participate in team-building games created by IBU’s HR assistant (psychologist). There were a total of 5 interactive games played with each motivating function.


Overall attendance for the training was 100% of total participants targeted. To involve the Puskesmas authorized about the activity, the training was also attended by the village-midwives from Puskesmas. During the training 2 village-midwives from 2 Puskesmas attended the training.




Posyandu Refreshment Training of Trainers
(TOT)
Based on surat edaran Menteri Dalam Negeri Nomor:411.1/1180/PMD 25 August 2000 on Posyandu Revitalization IBU are now in the role of assisting the health-providing activity conducted in Posyandu and Pustu of its working area. Main aimed of the activity is to improve Posyandu’s function as community based health providing system, where environments are encourage to their own health independency.





The activity was held in Jogjakarta Plaza Hotel on 28-30 March 2007 with Bantul DHO’s MCH division (the training was opened by Head of Bantul MCH division, drg. Dewi Marhaeni, MSi.) and Gadjah Mada University (Medicine and Psychology Faculty) assistance. We invited a total of 25 participants to attend the training which are from midwives, Pustu’s staffs, cadres and other village’s health authorized. And the topics regarding the TOT were:

1. ‘Community Health-Promoting Methodology'
Speaker: Dra. Yayi Prabandari, MSi, Phd. (Health Promotion Sub Division from Medical Faculty Gadjah Mada University

2. ‘Nutritional Importance for Children’s Growth and Development’
Speaker: dr. Endy Prayitno, Sp.A (K)-Nutrisi (Child Nutritionist and expert from Paediatric Division of DR. Sardjito General Hospital)

3. ‘Physiological Approach towards better Community Mobilization for Posyandus Monthly Activity’
Speaker: DR. MG.Adiyanti, M.S.( Lecturer from Master Degree of Psychology Faculty Gadjah Mada University Yogyakarta)

4. ‘Children’s illness identification through National Standard of MTBS (Manajemen Terpadu Balita Sakit)’
Speaker: dr. Setya Wandita, DHSM, M.Kes, Sp.A(K) (expert on basic essential neonatal care from Pediatric Division of DR. Sardjito General Hospital)

5. ‘Early Identification of High Risk Pregnancy’
Speaker: H. Risanto Siswosudarmo, dr. SpOG (Obstetric and Gynaecologic expert and Head of Specialist Study Program from Obstetric Division of DR. Sardjito General Hospital.

Support DHO’s Program
In the second week of March, Bantul DHO contacted IBU’s team to support their TV Broadcasting socialization of DB4MK (Desa Bebas 4 Masalah Kesehatan). The DHO ask for IBU’s participation to become spokes persons on NGO’s role in improving health for community in Bantul. The activity was broadcast live from Jogja-TV’s 1 hour interactive talk show ‘Dokter Kita’ on 18 March 2007. IBU sent the office’s Project Director (Destrian Setiawan) and Project Manager (Yudha Mathan S., dr.) to support the show. The show discussed on governmental main strategy in reducing 4 major health issues faced by Bantul DHO (maternal mortality, child’s mortality, malnourished children and dengue) and the collaborative action with NGO’s working in the area.

Overall attendance for the training was 100% of total participants targeted. To involve the Puskesmas authorized about the activity, the training was also attended by the village-midwives from Puskesmas. During the training 2 village-midwives from 2 Puskesmas attend the training.
